Patron label layout screen

When choosing the Layout type for patron cards, the window displays
"Bibliographic Data to Print: List Fields", with Data Fields already defaulted
to "Patron ID cards".  It appears to the user that one could choose which
fields to print (which would be great, if the user could choose patron fields
as well as bib fields!)

The patron data looks to be hardcoded.  
In short, the patron data cannot be edited.  The interface should hide
the bibliographic data selection if you choose a patron card type,
since that's pretty confusing."
