The 'Generate Exception to this Holiday' radio button should only appear for
repeatable holidays.  Right now the form allows you to create an exception for
a single holiday (not repeatable), and it allows you to create multiple
exceptions for the same day.

This bug was introduced when a "showTitle" input field was removed from the
form.  The javascript showHoliday function is still looking for the 'showTitle'
id.
