Patch sent 10/6/09 -- new sysprefs are HoldButtonConfirm,
HoldButtonPrintConfirm, HoldButtonIgnore.  The code doesn't require any of them
to be on, but sites should use at least one of HoldButtonConfirm or
HoldButtonPrintConfirm.
