Patch pushed to HEAD.  However, there's a glitch with the 'toggle all' action;
it should not be toggling the row and column header checkboxes.  If it toggles
one of them on, the end result will be that all libraries for that row or
column will have their transfer limit flag set.

If we use JavaScript to manage the row/column header checkboxes, it should
follow a rule that the header gets flipped only when all of the boxes in that
column or row are in the same state.
