I have done a comparison between the edit patron form on the OPAC and on the
staff client and we have some fields missing that patrons should be able to
edit.  And there are some formatting issues as well.

#1 - Opac needs other name field
#2 - permanent address should be alternate address
#3 - Country missing from alternate address
#4 - address 1 & 2 in a textarea instead of two textboxes
#5 - opac needs work email
#6 - alternate address needs email in the opac
#7 = should we show the alternate contact?
#8 - additional parameters aren't showing on the OPAC (I think they were at one
time)

Anything I'm missing?
