I'm expanding the scope of this bug to include the pop-up window which appears
when you click the "detail" link in the warning dialog. The window shows a form
with details of what I assume are the suspected duplicate. Why is it a form?
Why doesn't more information display? I would think address and phone number
belong here at the very least.
