Owen,
As per our discussion, this is replicated when the holding is created at the
same time as the bib.

Known workarounds at this time:
1) Use a different browser.
2) Create the bib but don't create the holding after saving.  Instead, search
for the bib and then add an item.  This will prevent creating a duplicate bib.
