Proposed additional fixes

- Correcting page title
- Adding breadcrums
- Adding sidebar menu (matching other batch mod pages)
- Showing "loading" graphic during AJAX operations
- Adding empty option to list of MARC tags so that onchange event
  will fire when the user is choosing 000
- Other structural markup changes

I still found that trying to make changes to 000 didn't work. "All operations
processed," but the status for each titles says "Nothing done."
