There is a problem when a comment is edited after first approval:

1) activate System preference reviewson 
2) add comment in OPAC
3) approve comment in staff client - comment is shown in OPAC correctly.
4) edit your comment
5) try to approve in staff client - comment is not shown in list of comments
waiting for approval.
