The patch does what it says. But there are 2 typos:

Two consecutive <b> tags here:
[% ELSIF ( filerror.OPNERR ) %]<b><b>ERROR:</b>

and a remaining WARNING here (one <b> tag should be removed also):
[% ELSE %]<b>WARNING:<b>ERROR:</b>
