All tests pass, perlcritic is unchanged.

Test plan
- Verify ExtendedPatronAttributes hides/shows new patron attribute
  search criteria
- Verify patron attribute show up correctly
  - pull down list for attributes linked to authorized values
  - input field for other attributes
- Verify search works correctly
