This patch changes loop handling in a single C4::Biblio subroutine, which does
keep it's scope narrow.  Testing shows that this has not broken anything, even
though the problem could not be replicated by the signer-off.  Adds numerous
comments about what's going on in this while loop, which will make any
subsequent revisions easier.
