Check line 95 in koha-tmpl/intranet-tmpl/prog/en/modules/cataloguing/

<ol><li><label for="title">Title: </label><input type="text" id="title" 
name="title" value="[% title %]" /></li>

My theory is if [% title %] starts with a " then you get value="" and there is
the problem.

So you could try adding [% title |html %]
