Calendar icon missing

- The calendar icons are not displayed, only text "Show calendar". The image
path points to (...)/koha-tmpl/intranet-tmpl/prog/en/lib/calendar/cal.gif (does
not exist). With other reports (e.g. Patrons), the path points to
(..)/intranet-tmpl/prog/img/famfamfam/silk/calendar.png

- While loading the page I get a JavaScript Reference Error: Calendar is not
defined. As a result, date picker does not work.
