As suggested by Matthias I have updated the patch to use
C4::Context->preference('OPACBaseURL') in place of $ENV{SERVER_NAME} within the
Auth_with_Shibboleth script.

This seems to work in my testing, though I didn't know how to reproduce the
issue Matthias highlights so couldn't test that it actually fixes the issue.
