Bug 3050: [QA Followup] Fix various QA issues, plus some minor improvements

QA followup for: Bug 3050 - Add an option to upload scanned invoices.

- Fixed various QA issues.
- Added unit tests for the new module.
- Improved error handling; extended pod documentation
in Koha::Misc::Files
- Attached file records were not properly handled when merging
invoices; fixed.

Test plan add-ons:

1/ prove t/db_dependent/Koha_Misc_Files.t
2/ try to merge some invoices with files attached; after merge,
all files previously attached to individual invoices being
merged should be attached to resulting invoice (merge destination)
