Some follow-up work to allow better error handling when a shibboleth session
exists but does not match a koha user, and allow for shibboleth login from the
front page in bootstrap should you not already be logged in.

I've also added the beginnings of a perldoc to the Auth_with_shibboleth.pm
module to guide configuration.
