Finally got this working with Martin's help! Woohooo!

Some first remarks:
- The link to log in with Shibboleth seems to only appear on the start page,
but not for example on the result list.
- It would be nice if Shib login also directed you to the patron account, like
the normal login does. But maybe it's safer to always redirect to the place the
user started from - in case someone logged in to place a hold or similar.

Tested ok:

>From start page:
- OK Shib login with correct username/password
- OK Shib logout with correct username/password
- OK Koha login
- OK Koha logout
