[SIGNED-OFF] Bug 12133: Guarantor requirements when registering a patron

Add two requirements when registering a new patron:

- A child patron must have a guarantor. This is controlled by
  a new syspref ChildNeedsGuarantor.
- A guarantor cannot be a guarantee.
