I can't do a video - but I found the reason:
"NetworkError: 404 Not Found -
http://localhost/cgi-bin/koha/svc/patron/show_checkouts_to_relatives"

This file is not added by this patch set, but referenced in opac-memberentry.tt
on line 829.
