After installing fresh version from debian repository with UNIMARC, no Fast Add
was present for me ... but i have finally removed my Fast Add configuration and
hidden a lot of fields in the default grid and all is working !

I will try to reinstall again Koha from debian package to see what could be
wrong with FA !
