Using Koha version 3.08.01.002

Returned books are being checked in (with on-screen confirmation), only to be
switched back to a checked-out status under the name of the last patron to
check out the book. Originally chalked it up to user error, but upon noticing a
large number of occurrences, realized that it is, indeed, some kind of glitch.
Our in-hour IT team has been unable to pinpoint the problem.
