Bug 5670: [QA Followup] Fix `div.patroninfo`.

* members/housebound.pl: Provide full patron hash keys, extended patron
  attributes & picture to template.
* koha-tmpl/intranet-tmpl/prog/en/modules/members/housebound.tt:
  Refactor to use new information.
