I like it.  It is very similar to my request.  The one thing I have trouble
with is the note.  This does not give the option to place individual notes on
the individual items.  See

suggestion and let me know your thoughts.

I still think the real estate could be consolidated more by combining the
dropdown and process into an action button that defaults to payment.  What do
you think?
