Bug 21233: Add Koha::Exceptions::Password

This patch adds some exceptions that we need. To test:

- Apply this patch
- Run:
  $ kshell
 k$ prove t/Koha/Exceptions.t
=> SUCCESS: Tests pass!
- Sign off! :-D
