I have done the following changes to the patches on this bug report to try and
get it moving along again:

(#1 and #2 address Katrin's notes in comment 80)

1. Amended the first patch to include a proper test plan

2. Fixed the commit messages of all commits to be in the format: (Bug: XXXX)

3. Rebased all commits against master

4. Reworked the css in the second patch because since that commit was written
we have switched to scss.

5. Added an additional patch which fixes a few bugs when placing an item level
hold from the OPAC. 

I will now attach these commits to the bug report.
