Bug 21475: Fix crash on missing default itemtype

Test plan:
Enable ArticleRequests.
Find book without itemtype (942c). Maybe you need this:
  update biblioitems set itemtype=NULL where biblionumber=[...]
Goto opac detail for that book. No crash anymore?
