Okay, so this became a bit bigger set of changes and I'm not sure how to ever
get this in since I can imagine the last patch rotting away like no other.
Anyway, this is an attempt to use an include file for any time a biblio title
is to be displayed. There are separate includes for the title field in head and
any other stuff in html. There are also classes added to each part. This allows
one to style them independently and to e.g. easily hide part information from
breadcrumbs if that's necessary.

I've tried to find all occasions where the title is displayed, and I've also
tried to test them, but there may well be something that I've missed.
