Removed staff-global.css changes.

Katrin, I've also added the biblio-title include to places where it's logical
to use it and wasn't before. E.g. it makes no sense to display subtitle for
checkouts but not for article requests, so I've done my best to unify the
related displays. Same with e.g. the sub-pages of catalog record (holds,
article requests, checkout history etc.).
