This was opened in 2016, so good luck with that.  It IS a complicated issue to
fix right.

The patch allows librarian to move ahead when this occurs whereas a popup will
just tell them they are not crazy, but leave them helpless again.

In our production environments, it has occured a few times (to much frustration
and bewilderment, and never a complaint since we put the patch on, even though
the discremencies in the UI a quite obvious).
