Excellent work, this looks like a great base for future customization.

Two things:
1 - Many qa script failures - missing filters and a critic complaint on XSLT:
 FAIL   C4/XSLT.pm
   FAIL   critic

violation(s).  

2 - I don't see the new option in lists - it works on detail and cart
