I first thought this could be Bug 22550 - OPAC suggestion form doesn't require
mandatory fields, but this is in 18.11

I have also checked that it works on our 18.11.11.

I agree with Jonathan that a JS error is a likely candidate for causing this
issue. If you have anything in OpacUserJS try emptying out the preference and
see if that would fix your issue.
