Julian, what's missing to have this ready for push?
Can you provide a config for apache?

If I understand correctly, we are not modifying any existing Koha core files
(there are very few changes on bug 20630 that should be safe) and so we could
have this pushed to be tested by developers, when production sites would
continue to use the standard plack architecture. It that correct?
