I squashed all patches, added the missing license headers, and made small
changes to emphasize the fact that opac and intranet are two different apps.
This makes the patch smaller and removes one of the FIXMEs.
I also had to fix OPAC login when running app.psgi with starman (see comment in
Koha::App::Plugin::CGIBinKoha::_psgi_env)

Back to needs signoff.
