As this is an entirely new notice this cycle, I'd be tempted to update the
default to check for the presence of 'reason' and give a slightly different
form if it is not present.... as a side note, if we're going to allow
notifications without a reason, then perhaps rather than a checkbox we should
instead have 'Confirm', 'Confirm and notify` and `Cancel` buttons/links to
close the modal?
