Hi Jeff, 

this is the Koha project - an Open source library system.
>From your screenshots and the description I am not sure you meant to file the
bug here. We don't ask for "I have read Bugzilla Etiquette and the Mozilla
Community..." here.

Our test reads:

"To create a Bugzilla account, all you need to do is to enter a legitimate
email address. You will receive an email at this address to confirm the
creation of your account. You will not be able to log in until you receive the
email. If it doesn't arrive within a reasonable amount of time, you may contact
the maintainer of this Bugzilla installation at chris at bigballofwax.co.nz."
