This is blocking bug 24608 and I don't know how to move it on.. I've answered
the questions and asked for feedback if the answer isn't understood.. so much
time has passed that without going back to the begging of all these issues I
can't really give more detail...

But 24608 highlights the issues we have by not properly handling offsets and
this bug resolves all those issues whilst maintaining existing compatibility.
