Sorry for not being more specific.  The problem happens when a patron uses the 
online catalog to search for a title and then tries to reserve that title using 
the "reserve this title" button.  After the patron logs into Koha, the 
page "reserve page for Item:xxx" claims that there is already one reserve on 
the item.
