Maybe this is related to another bug I found, shown at

Fix: Add the line
  SetEnv KOHA_CONF /path/to/koha.conf
in the koha opac httpd.conf section.  I have filed a bug report for this
and will fix it in the next 3 days.

Let me know if it works for you.  Basically, koha is trying to open
the include files from / instead of the right path.
