[Koha-devel] RFC - installer changes

I revised my mod_rewrite rules for an install without virtual hosts.  
The following seems to work.

1. Install koha in single mode under $ROOT/koha where $ROOT is the  
location from which files are served.
2. Create the file $ROOT/.htaccess file with the following rules
