OK, I'm a bit late to this, but in case it's still useful for someone,
the most up-to-date version is living here:

http://git.catalyst.net.nz/gw?p=koha.git;a=tree;f=import/csv;h=3dcf435529baf255bf371365dd4de6a6ceb1a108;hb=import_branch

the important one is csvtomarc.pl, but there are other migration utils
and examples and so forth in there as well.
