I'd like to suggest some modifications:

Developer: Name of JQuery developer
Creation Date: the date you entered the script (YYYY-MM-DD)
Purpose: Purpose of the JQuery script
Status: Completed / In progress
Intranet or OPAC?:
Version: (The Koha version you are using this with, or Any, if applicable)
