Are you authenticating against Active Directory or OpenLDAP, or some other
LDAP server?

Maybe these should both be 1?

    <replicate>0</replicate>  <!– add new users from LDAP to Koha database –>
    <update>1</update> <!– update existing users in Koha database –>

Apart from that, I have no idea really what the problem may be...
