Further to this, 

I was seeing segfaults, so while investigating this, I took the
opportunity to migrate from Xen to KVM.
Linode is migrating all their nodes, I'm hoping that this fixes the
issue (which did look like underlying hardware ones)

If not, it won't hurt any
