I have pushed this patch.  Since this is your first commit accepted
into the main tree, let me welcome you to the Koha committer cabal!

A couple comments about this patch:

* my XHTML validator says that there's an extra </div> - could you
chase that down?
* as I mentioned before, it would be nice if the form remembered the
modules that were selected and and displayed the mutliselect control
accordingly.
