[Koha-patches] [PATCH] Change to how email link is displayed for on-hold items.

I wonder if anyone has any comments on this change, since the patch
seems to be languishing? It's a no-brainer for my library, not least
because that's how it worked in our previous Koha install. The idea is
to prevent the librarian from sending a notification email before the
item is at the destination branch (something that happened frequently
when we first started sending email notifications).
