[Koha-win32] Server 500 Errors

In my attempt to modify the subfields for tag 100 Personal Name per instruction, I encountered the following error message.  Can someone help?

 

The server encountered an internal error and was unable to complete your request. 

Error message: 
Premature end of script headers: marc_subfields_structure.pl
