[Koha-win32] Data no fill in serials module

Dear all
Firstly I have installed koha ver. 2.2.8 with following configurations:
Microsoft window 2000. Apachi 2.0.59, Mysql 5.0 & Perl 5.8.8 build 817. I
fill all information in, Add a Subscription form then I click Add
Subscription but only filed names are showing not field values.

I am facing this error anybody know this error please help to solving the
problem
