Hi Simon, are you self-hosting or do you work with a support provider? 

Bugzilla is used to report bugs and enhancements to the Koha Community. This
sounds like something needs to be checked out on your server, for example the
SIP server might need restarting.

But you should/need to raise the issue with someone who can get on your server.
