You forget to delete the password reset entry after resetting the password.

You mean from table borrower_password_recovery ? I validated it does delete the entry. And I can't reproduce the 404 of your next comment. When I append your line to my opac base url, it sends me to a message: " We could not authenticate you as the account owner. Be sure to use the link you received in your email.
